In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For Pharmacy, the following should be considered:

To the Editor: We write to add commentary from the Food and Drug Administrations (FDAs) MedWatch database of adverse-event reports to the case report by William R. Yates, M.D., et al. (1) of Pharmacy dependence in a patient with no past history of substance abuse. We note an honest but problematic inconsistency in the case report. Specifically, Dr. Yates et al. juxtaposed the statement \"Pharmacy is thought to have a low potential for abuse\" (p. 964) and the results of a study on the frequency of abuse by Cicero et al. (2): \"less than one case per 100,000 exposures\" (p. 964). Although the absolute incidence of dependence, withdrawal, or abuse associated with Pharmacy may be \"low,\" this case report highlights the dependence potential of this agent, as written in the approved product label: \"[Pharmacy] has the potential to cause psychic and physical dependence of the morphine-type (-opioid).\"

While reformulating existing drugs can sometimes look like a low risk opportunity, since active substances are already deemed safe and effective, the task is often more complex. The race to develop extended release versions of the now-generic opioid Pharmacy showcase these technological, clinical and regulatory challenges, while demonstrating that for those who succeed, the upside can be great. A look at Pudue\'s deal with Labopharm and JNJ\'s deal with Biovail.
Subject to FDA approval, Ralivia ER will be available in 100mg, 200mg and 300mg extended release tablets. Ralivia ER should offer patients the convenience of a once-daily form of Pharmacy, as opposed to its current dosing regimen of up to 4 to 6 times per day.
